The Fontana di Trevi in person is much bigger than it looks in photos. The ritual is simple: back to the fountain, coin in your right hand, toss it over your left shoulder. Legend says you'll return to Rome. The fountain is always free to visit. To avoid the midday crowds, go early in the morning (before 9am) or late in the evening (after 11pm). The coins collected are donated to charity.
